📢📢 Help me Help you. There’s at least one or more of these per shift received like this in the laboratory. A stat specimen but no label. Unfortunately we can never run these due to patient safety.

But how can we prevent these errors and unnecessary redraws? Well, patients for one can make sure that the person who is drawing their blood labels their tube at bedside. This is not overstepping, this is advocacy. This is empowerment. The more you know.

Things run differently in the laboratory

The College of American Pathologists (CAP) requires a strike-through with initials to protect the integrity of patient records.

Here’s the real reason behind it:

🔍 1. Transparency (no hiding errors)

A single line through the mistake keeps the original entry visible.
This shows exactly what was written, what was corrected, and that nothing was erased or altered secretly.

✍🏽 2. Accountability

Adding your initials (and often date/time) ties the correction to a specific person. That means anyone reviewing the record knows who made the change.

⚖️ 3. Legal & regulatory protection

Lab records are legal documents. If something is ever audited or questioned:

* A white-out or deletion looks suspicious ❌
* A strike-through with initials shows proper, compliant correction ✅

🧪 4. Patient safety

Accurate traceable documentation ensures:

* No confusion about results
* No risk of using incorrect or altered data
* A clear history of what happened

💡 What CAP expects (simple rule):

* One clean line through the error
* Write the correct info nearby
* Add **initials + date/time (if required by your lab policy)**
* Never use white-out or completely erase entries

Doctors see what’s on the (outside) we see what’s going on (inside). Believe it or not, we alert them when it’s time to stop or lower the medication.

Ever heard of PT or aPTT? Probably not. But Medical Laboratory Scientists run these tests every day to make sure your blood clots when it should and stops bleeding when it has to.

“PT/INR checks the extrinsic pathway.
aPTT checks the intrinsic pathway.
Medical Laboratory Scientists check both… because your life depends on it. 🩸”

“You may never see us but when you’re bleeding, surgeons and doctors are waiting on our PT and aPTT results. Medical Laboratory Scientists keep the clotting system in check”

No Medical Laboratory Scientists = big problems.
