Cervical Dilation: How the Body Opens for Birth

Cervical dilation is the gradual widening of the cervix—the opening at the base of the uterus that leads to the va**na. During labor it progresses from closed (0 cm) to 10 cm, creating a passage for the baby to be born. Tracking dilation helps clinicians understand how labor is unfolding and what support might be needed.

The Phases of Dilation

1) Early labor (0–3 cm)

The cervix begins to soften, thin (efface), and open a little.

Contractions are usually mild and irregular, about every 5–30 minutes, lasting 30–45 seconds.

Especially for first-time moms, this phase can be quite long—sometimes many hours or even a couple of days.

2) Active labor (4–7 cm)

Dilation picks up speed.

Contractions grow stronger and more regular, roughly every 3–5 minutes for 45–60 seconds.

Many labors spend 4–8 hours in this phase.

3) Transition (8–10 cm)

The most intense—but typically the shortest—stretch before full dilation.

Contractions can come every 2–3 minutes, lasting 60–90 seconds.

Often lasts 30 minutes to 2 hours and may bring shaking, nausea, or strong emotions.

4) Full dilation (10 cm)

The cervix is completely open; the pushing stage can begin.

Baby moves down through the birth canal for delivery.

What Can Influence Dilation Speed?

Birth history: Prior va**nal births often mean faster dilation.

Contraction effectiveness: Strong, coordinated contractions help the cervix open.

Baby’s position and size: A well-positioned head (occiput-anterior) tends to facilitate progress.

Environment and emotions: Fear, tension, or feeling unsafe can slow labor; calm, continuous support can help it advance.
